Medical Updates

It is vitally important that we know all particulars about your medical history so that we know how to respond to your medical needs while undergoing dental treatment. For example, if you have had rheumatic fever, or perhaps your doctor told you you have a prolapsed heart valve, you would need to be premedicated with an antibiotic before having your dental cleaning. If you are diabetic, we would want to make sure you have eaten before dental treatment, since you may not be able to after - it is important to maintain a proper blood sugar level.

Therefore, we ask that you update our medical history form on a regular basis and notify us of any changes to your health. If you take certain medications, be sure to bring us the list of medicines and what you take them for. We also need to know if you have specific allergies to either foods or medications. Our medical form lists all the required areas of importance. Please take the time to fill it out properly and sign and date the form.
